Yeah this tut is awesome. This might be a "newbie" question but, How to you make the spiral/swirl around the baby's arm.

You need to use the Pen Tool. [P] on the keyboard. He made a line wrap around with that, and then set it to brush with a 1-2px brush set to a color of choice.

I am still not understanding how do use the pen tool. Everytime I use it, it just makes a box and I don't know what to do after that.

There's a bunch of settings you have to fix to get rid of that box stuff, I'll look for the tutorial for you.
